The ICC SDP Expressway is an innovative community-based initiative designed to help individuals and families successfully transition into California’s Self-Determination Program (SDP) through structured guidance, education, coaching, and peer support.

Developed by the Integrated Community Collaborative (ICC), the Expressway model helps families navigate what can often be a complex and overwhelming process by providing clear pathways, personalized support, and practical tools every step of the way.

Our goal is simple: reduce barriers, increase participation, and ensure families have the information and confidence needed to make informed decisions about self-determination.

The ICC SDP Expressway is a community-centered support model that helps individuals and families:

  • Understand the Self-Determination Program
  • Navigate the transition process
  • Prepare for orientation and enrollment requirements
  • Develop person-centered plans
  • Understand budgets and spending plans
  • Connect with Independent Facilitators and community resources
  • Build confidence and self-advocacy skills
  • Successfully launch and sustain their SDP journey

Through one-on-one coaching, workshops, office hours, and peer-to-peer support, families receive practical assistance from individuals who understand both the system and the lived experience of navigating it.

Many families face significant challenges when attempting to transition into the Self-Determination Program, including:

  • Language and cultural barriers
  • Lack of information and awareness
  • Difficulty understanding SDP requirements
  • Challenges navigating Regional Center processes
  • Limited access to individualized guidance
  • Fear of making mistakes during the transition process

The ICC SDP Expressway was created to address these barriers by providing culturally responsive, family-friendly support that meets families where they are.

Lear

Families receive information about SDP, eligibility requirements, benefits, responsibilities, and available supports.

Prepare

Participants receive guidance on orientation requirements, documentation, timelines, and transition planning.

Plan

Families are supported through person-centered thinking and planning processes that focus on the individual’s goals, strengths, preferences, and vision for the future.

Build

Participants learn about budgets, spending plans, service options, and community resources available through SDP.

Launch

Families transition into SDP with confidence, support, and ongoing access to community resources and peer mentorship.

VMRC SDP Expressway Pilot Program

ICC is proud to partner with Valley Mountain Regional Center (VMRC) in implementing the SDP Expressway Pilot Program. This innovative initiative focuses on accelerating successful transitions into Self-Determination through education, coaching, outreach, and community engagement.

The pilot is designed to:

  • Increase SDP enrollment
  • Improve family preparedness
  • Reduce transition barriers
  • Promote equitable access
  • Strengthen community partnerships
  • Improve participant outcomes

Lessons learned through this pilot will help inform future efforts to expand Expressway services throughout California.
