The Integrated Community Collaborative (ICC) Men’s Group, known as “Alianza de Hombres” or “Men’s Alliance,” plays a vital role in informing, empowering, unifying, and engaging men who have family members with intellectual and/or developmental disabilities.

The group’s objectives are designed to make a substantial impact:

  1. Inform: Alianza de Hombres provides essential information to its members about the challenges, resources, and rights related to intellectual and developmental disabilities. This knowledge equips them to advocate effectively for their family members.
  2. Empower: By offering support, resources, and a sense of community, the group empowers men to take an active role in advocating for their loved ones with disabilities. Empowerment can lead to more effective advocacy and support.
  3. Unity: Alianza de Hombres aims to create a sense of unity among its members. Through shared experiences and a common purpose, men can find strength in numbers and work together to address the needs of their family members.
  4. Local Chapters: The establishment of local chapters in various communities is a strategic approach. It allows for grassroots leadership development and ensures that the group’s impact can reach a broader audience across California.
  5. Meetings and Trainings: Regular meetings and training sessions provide a platform for members to exchange ideas, learn from each other, and stay updated on relevant issues and advocacy strategies.

ICC SDP Independent Facilitator

The Integrated ICC Independent Facilitators play a crucial role in supporting individuals participating in the Self-Determination Program (SDP). Here’s how our trained Integradora Independent Facilitators assist families on their SDP journey:

 

  • Informed Decision-Making: Our Independent Facilitators help individuals make well-informed decisions regarding their individual budget. This includes understanding the options and choices available within the SDP framework.

 

  • Service Coordination: We assist in defining the various services and supports that align with the participant’s Individual Program Plan (IPP). This ensures that the chosen services are in line with the individual’s unique needs and goals.

 

 

  • Needs Assessment and Planning: Identifying immediate and long-term needs is a critical aspect of our support. We work hand-in-hand with families to develop options to meet these needs effectively, promoting the individual’s well-being and independence.

 

  • Person-Centered Planning: Our Independent Facilitators actively participate in, lead, and advocate for participants during the person-centered planning process. We collaborate in the development of the IPP, ensuring it accurately and culturally reflects the individual’s preferences and goals.

 

  • Accessing Services and Supports: We help individuals obtain the services and supports they’ve identified in their IPP, ensuring that unmet needs are addressed and that the transition to SDP is as seamless as possible.

 

Our Integradora Independent Facilitators bring a wealth of knowledge and lived experience to the table. This combination of expertise and empathy allows us to guide families through the SDP transition, identify potential barriers, and ensure that individuals can fully engage and participate in the program. Our ultimate goal is to support individuals in achieving meaningful and self-directed lives within the SDP framework
